Includes controller and original USB dongle for connectivity, both working perfectly. Controller hasn't been used too much - there is some minor signs of usage that I have tried to picture, but overall great condition and well looked after (still has that "new plastic" smell!) Does not include case or other accessories but will be well packaged.
