Antique Cast Iron 13x16 Arch Top Gothic Heat Grate Register VTG Black Old 91-25B
The manufacturer is unknown, its marked patented Jan 1940. It's painted in a black finish. The louvers are complete but one does not function as the operator arm is broken as shown in last picture.
