This iPod touch is 5th gen 64gb A1412 in space grey. It has been factory reset and appears to work without issue however the screen is coming away. I suspect this is because the battery has expanded and needs to be replaced so this is a project for someone handy with a soldering iron. There are instructions on the ifixit website and 3rd party batteries are available and affordable.
